Medical Record Requests

Please fax a medical records form located on our Patients webpage to (512) 814-1565.

Medication Refill Questions

If you need a prescription refill, please contact your pharmacy.  The pharmacy will send us a fax or electronic request for your refill.  Please allow 48 hours for a routine refill.  If you have a prescription related question, please call our office during normal business hours, Monday through Friday, 8:00am to 5:00pm at (512) 320-1500 for our Central offices and at (512) 832-0999 for our North offices.
