SMS Text Message Communication Consent
Capital Nephrology Medical Group offers text message (SMS) communication to patients enrolled in our Chronic Care Management (CCM) and Transitional Care Management (TCM) programs as part of their ongoing care coordination.
How Patients Opt In
Patients provide consent to receive SMS messages at the time of enrollment in our care coordination program. Consent is obtained directly from the patient during a clinical encounter or enrollment conversation, and is documented in the patient’s medical record. Enrollment is voluntary, and patients may decline SMS communication while still receiving all other program services.
Message Frequency
Message frequency varies based on each patient’s care plan and needs. Patients typically receive periodic check-in and care coordination messages each month.
Cost
Message and data rates may apply, depending on the patient’s mobile carrier and plan.
How to Opt Out
Patients can opt out of SMS communication at any time by replying STOP to any message. After opting out, no further text messages will be sent. Patients may also request help by replying HELP, or by contacting our office directly.
Privacy
Capital Nephrology Medical Group protects patient information in accordance with HIPAA and applicable privacy laws. Mobile information and consent are used solely for care coordination and are never sold or shared with third parties for marketing purposes. Please see our Privacy Policy for details.
